Macmillan Coffee Morning

Here at Springwood Surgery we were delighted to host a coffee morning in support of Macmillan. We had lots of delicious home bakes made by our members of staff and prizes donated for a tombola along with some fantastic raffle prizes from local businesses. With the support of our wonderful patients we managed to raise £215 on the day.

Please look out for our next fundraising event in December, next time in support of Teesside Hospice.

Surgery Closure

11th October 2018

Springwood Surgery will be closed from 12.00 pm on Thursday 15th November at 12:00 hours until Friday 16th November at 08:00 hours due to staff training.

The Great North Care Record

23rd February 2017

All patients registered with a GP Practice in England have a basic Summary Care Record (unless you have opted out). This record shows the following information about your health:

  • Medicines you are taking
  • Allergies
  • Bad reactions to medications

All patients (apart from the patients that have opted out of the basic summary care record) will be given a Great North Care Record. This will allow Hospitals, Ambulance Service, Mental Health Service, Out of Hours Service and Urgent Care to see medical information about you such as:

  • Medical Conditions
  • Medication
  • Operations and Treatment
  • Tests and Results
  • Contact details for your next of Kin

If you want to Opt-Out of having a Great North Care Record please ask a member of the Reception Team for an Opt-Out Form, complete it and return it to the reception team.

Please Note: If you decide to Opt-Out of the Great North Care Record your medical record will not be shared with the scheme but it will not be shared with any Local Community Services or the Out of Hours Service too.

Additional Information

23rd February 2017

You can now add the option to share Additional Information to your Summary Care Record if you have one. This will enable services such as, Clinicians, Hospitals, District Nurses, Physiotherapy and the Out of Hours Service to view additional information about you. The Information they will see will be:

Long term health conditions
Heart Problems
Rare medical conditions

Relevant Information
Clinical Procedures
Why you need a certain type of medication
The care you are currently receiving

Healthcare needs
Communication needs
Long-term conditions
Legal decisions about your care

Vaccination details

If you would like to add the option to allow addition information to be added to your Summary Care Record please ask for an “Opt-In” Form from reception. You will need to complete it and return it to the reception team.

Friends and Family Test

We would like to thank all our patients that took the time to complete a Friends and Family Test, the on-going results have been extremely positive with 98% of patients stating that they would be “Very Likely” or “Likely” to recommend our surgery to a Friend or Family Member.

We have addressed some of the comments made by the 2% negative comments and agreed an Action Plan with our Patient Group to address these issues.
